New Patient Registration

doctor_examining_earThe doctors welcome new patients who live within our practice area. 

To register with the practice please call at reception with your medical card if possible. We will complete your registration form with you and make you an appointment to see our practice nurse for a new patient registration health check.

Your details will be sent to the Primary Care Trust who will then issue you with a new medical card and will arrange for your medical records to be sent to us.

Please notify us as soon as possible if you change your name or address so that we can ensure that your details are up to date.

Please contact reception for further information.

Temporary Residents

If you are staying in our practice area and need to see a doctor or nurse we will be pleased to make an appointment for you. Please call at reception or telephone us on 01229 772123.

PLEASE NOTE: For one afternoon per month we focus on staff development and training, and during this time the surgery will be closed. If you require medical assistance during this time please dial 03000 247 247.  Below is a list of planned afternoon closure dates, the surgery closes at 13:00:

TUESDAY 14TH MAY 2013, WEDNESDAY 12TH JUNE, THURSDAY 19TH SEPTEMBER, WEDNESDAY 16TH OCTOBER, TUESDAY 19TH NOVEMBER AND THURSDAY 5TH DECEMBER 2013.
